Others may speak to it, cradle it, adjust its clothing or simply enjoy having it nearby. Some may become more communicative or appear calmer.<\/p><p>Others may show absolutely no interest.<\/p><p>And that is perfectly valid.<\/p><p><strong>The goal is not to make someone interact with a doll. The goal is to discover whether the interaction is meaningful to that individual.<\/strong><\/p><p>Doll therapy belongs to a broader group of non-pharmacological approaches used in dementia care. These approaches focus on supporting emotional well-being, engagement and quality of life without relying solely on medication.<\/p><p>Research has reported promising results. A systematic review involving seven studies and 295 participants found improvements in emotional state and communication and reductions in some challenging behaviors. A person who rarely initiates conversation may begin speaking. Someone experiencing restlessness may become more settled. Another person may simply smile while holding the doll.<\/p><p>Research has explored these responses through concepts such as attachment and caregiving behavior. In one randomized controlled trial involving 134 women with moderate-to-severe dementia living in nursing homes, participants receiving a structured doll therapy intervention showed reductions in behavioral and psychological symptoms compared with a control intervention, while also demonstrating greater caregiving and exploratory behaviors toward the doll.\u00a0<\/p><p>But perhaps the easiest way to understand the idea is not through statistics.<\/p><p>Imagine that someone has increasing difficulty telling you:<\/p><p><em>I feel lonely.<\/em><\/p><p><em>I need something familiar.<\/em><\/p><p><em>I want to take care of something.<\/em><\/p><p><em>I need reassurance.<\/em><\/p><p>A meaningful activity may sometimes allow an emotion to be expressed even when finding the right words has become difficult.<\/p><p>The doll is not necessarily important because of what <strong>we<\/strong> think it represents.<\/p><p>What matters is what the interaction appears to mean to <strong>that person<\/strong>.<\/p><h2>Emotional Connection Can Remain When Memory Changes<\/h2><p>One of the most important lessons in dementia care is that abilities do not simply disappear all at once.<\/p><p>Someone may struggle to remember a recent conversation but still respond to a familiar song.<\/p><p>A person may have difficulty naming family members but experience warmth when seeing them.<\/p><p>Someone may no longer be able to explain why an activity matters to them and still clearly show enjoyment, comfort or interest while doing it.<\/p><p>That is why meaningful dementia care cannot focus exclusively on asking:<\/p><p><strong>\u201cWhat does this person remember?\u201d<\/strong><\/p><p>We also need to ask:<\/p><p><strong>\u201cHow does this person feel?\u201d<\/strong><\/p><p>What brings comfort?<\/p><p>What creates engagement?<\/p><p>What encourages communication?<\/p><p>What helps this individual feel safe, useful or connected?<\/p><p>For one person, the answer might be music.<\/p><p>For another, gardening.<\/p><p>It could be photographs, folding familiar items, art, gentle movement, spending time outdoors or interacting with an animal.<\/p><p>And for some people, it may be a doll.<\/p><p>That does not mean they have \u201cbecome children again.\u201d<\/p><p>It means an activity has produced a meaningful response.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-98b5e10 e-flex e-con-boxed e-con e-parent\" data-id=\"98b5e10\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-3842a83 elementor-widget elementor-widget-heading\" data-id=\"3842a83\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">\u201cBut Isn't Giving an Older Adult a Doll Infantilizing?\u201d<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-b08ee41 e-flex e-con-boxed e-con e-parent\" data-id=\"b08ee41\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-e1f2ff4 elementor-widget elementor-widget-text-editor\" data-id=\"e1f2ff4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>This is probably the most important\u2014and most understandable\u2014question surrounding doll therapy.<\/p><p>The concern deserves a serious answer.<\/p><p><strong>A person living with dementia is still an adult.<\/strong><\/p><p>Their diagnosis does not erase their life story.<\/p><p>It does not erase the relationships they have built, the people they have loved, the work they have done, their personality or their right to be treated with dignity.<\/p><p>For this reason, infantilization and dignity are genuine ethical issues discussed in the professional literature on doll therapy. Researchers have considered the intervention through principles including autonomy, well-being, dignity and the possibility of causing distress.\u00a0<\/p><p>How the intervention is used therefore matters enormously.<\/p><p>A doll should not simply be pushed into someone&#8217;s arms.<\/p><p>A person should not be instructed to \u201cplay.\u201d<\/p><p>They should not be spoken to as though they were a child.<\/p><p>And a dementia diagnosis should never be used as a reason to assume that someone wants a doll.<\/p><p>Instead, caregivers can introduce an opportunity and observe the person&#8217;s verbal and non-verbal response.<\/p><p>If there is interest, that interest can be explored.<\/p><p>If there is rejection or discomfort, it should be respected.<\/p><p><strong>Person-centered dementia care begins with the person\u2014not the activity.<\/strong><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-00960a2 e-flex e-con-boxed e-con e-parent\" data-id=\"00960a2\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-2a01c87 elementor-widget elementor-widget-heading\" data-id=\"2a01c87\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Does Someone Have to Carry the Doll All Day?<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-a9cf2e5 e-flex e-con-boxed e-con e-parent\" data-id=\"a9cf2e5\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-4b66072 elementor-widget elementor-widget-text-editor\" data-id=\"4b66072\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>No.<\/p><p>This is another common misconception.<\/p><p>Using doll therapy does not necessarily mean giving someone a doll and expecting them to carry it throughout the day.<\/p><p>It can be a <strong>structured or occasional therapeutic interaction<\/strong>.<\/p><p>A caregiver or therapist may introduce it during a particular activity or at a moment when engagement could be beneficial. The person&#8217;s response can then guide what happens next.<\/p><p>Research itself has examined structured interventions rather than simply assuming permanent access to a doll. For example, controlled studies have used defined sessions led by caregivers or trained staff.\u00a0<\/p><p>Some individuals may develop a positive attachment and voluntarily want the doll nearby more often. Perhaps it evokes nurturing behavior. Perhaps it simply creates a peaceful moment shared between two people.<\/p><p>In that sense, the doll can sometimes function as a <strong>bridge to human interaction<\/strong>.<\/p><p>It should never become a replacement for it.<\/p><p>A person living with dementia still needs meaningful relationships, conversation, appropriate stimulation, movement, companionship and genuine human presence.<\/p><p>Good dementia care is not about placing an object in someone&#8217;s hands and walking away.<\/p><p>It is about discovering <strong>what helps that individual engage with the world around them<\/strong>.<\/p><h2>What Does the Research Say About Doll Therapy?<\/h2><p>The evidence is encouraging, but responsible dementia care requires us to describe it accurately.<\/p><p>A 2022 systematic review concluded that doll therapy appeared to improve emotional state and communication while reducing some disruptive or aggressive behaviors among people with dementia. However, the researchers noted limitations in the available studies and called for larger randomized trials with stronger methodology.\u00a0<\/p><p>A later systematic review and meta-analysis, published in <em>Geriatric Nursing<\/em>, evaluated ten studies involving older nursing-home residents with dementia. Researchers reported improvements in behavioral symptoms including agitation, apathy, irritability and wandering, as well as psychological outcomes such as pleasure, anxiety and depression.\u00a0<\/p><p>There was, however, an important finding:<\/p><p><strong>The analysis did not find a significant improvement in cognition.<\/strong>\u00a0<\/p><p>That distinction is essential.<\/p><p>Doll therapy does not reverse Alzheimer&#8217;s disease.<\/p><p>It does not restore lost memory.<\/p><p>It does not replace medical care.<\/p><p>And it is not appropriate for every person living with dementia.<\/p><p>Its potential benefit is different: helping some individuals experience <strong>comfort, engagement, communication or emotional well-being<\/strong>.<\/p><p>That is valuable\u2014but it is important not to promise more than the evidence supports.<\/p><h2>What If the Person Doesn&#8217;t Want the Doll?<\/h2><p>Then we listen.<\/p><p>A resident might ignore it.<\/p><p>They might push it away.<\/p><p>They might say no.<\/p><p>They might appear uncomfortable.<\/p><p>Or they may simply be uninterested.<\/p><p><strong>That response deserves exactly as much respect as enthusiasm.<\/strong><\/p><p>It does not mean that the person has \u201cfailed\u201d the activity.<\/p><p>It means we have learned something about that person&#8217;s preferences.<\/p><p>Perhaps music creates a stronger connection.<\/p><p>Perhaps they enjoy gardening, walking, painting, conversation, photographs or gentle exercise.<\/p><p>Person-centered care is not about finding one intervention that works for everyone.<\/p><p>It is about having enough curiosity to discover <strong>what works for this person, today, in this moment<\/strong>.<\/p><h2>Dignity Must Always Come First<\/h2><p>Behind behaviors associated with dementia there can sometimes be a need that is difficult to communicate.<\/p><p>A person may be uncomfortable.<\/p><p>Afraid.<\/p><p>Lonely.<\/p><p>Bored.<\/p><p>Overstimulated.<\/p><p>Looking for reassurance.<\/p><p>Wanting movement.<\/p><p>Or wanting to feel useful and connected.<\/p><p>Not every behavior needs to be \u201cstopped.\u201d Sometimes it needs to be understood.<\/p><p>That is where therapeutic activities can become valuable.
